Caring for a loved one with Alzheimer’s disease or another form of dementia can be overwhelming. From managing medical needs to ensuring a safe home environment, patients and their families often need additional guidance and hands-on support.
Doctors House Calls helps bridge the gap between specialized dementia care and medical care at home. Through our relationship with MedBetter Health PA, a participating provider in Medicare’s GUIDE Model, we help eligible patients access dementia care coordination, caregiver resources and in-home assessments.
Together, we help families navigate dementia care while supporting patients who wish to remain in their homes and communities.
What Is the Medicare GUIDE Program?
GUIDE stands for Guiding an Improved Dementia Experience. Developed by the Centers for Medicare & Medicaid Services (CMS), this voluntary care model is designed to support people living with dementia and their caregivers.

$2,625
per year in covered respite services*
Respite Care
≈76 hours of in-home care
19 four-hour visits
*Qualifying patients may receive up to $2,625 annually in covered respite services, equivalent to approximately 76 hours of in-home care, or 19 four-hour visits, based on Medicare’s national base rate for July 2026 through June 2027. Actual availability depends on eligibility, remaining benefits and the type of respite services used.
Covered GUIDE services are provided without patient cost-sharing for eligible, enrolled individuals.
Bringing GUIDE Support Into the Home
While many GUIDE services can be delivered virtually, in-person assessments and medical care remain important for patients who have difficulty traveling to appointments. Doctors House Calls provides hands-on, in-home support as part of its working relationship with MedBetter Health.
Our clinical team conducts designated in-home safety and fall-risk assessments for GUIDE patients within our service area, helping identify environmental hazards, mobility concerns and other factors that may affect a patient’s safety at home.
For patients who also need ongoing medical attention, Doctors House Calls offers separate home-based primary care services, helping individuals who are homebound or have difficulty accessing traditional medical offices. These services complement GUIDE’s dementia care coordination and caregiver resources.
Patients may retain their existing primary care physicians, and medical services outside GUIDE are billed separately under applicable Medicare coverage.

MedBetter Health administers the GUIDE Model, including eligibility reviews, enrollment, care navigation, caregiver education, 24/7 support and coordination of qualifying respite services.
Local In-Home Clinical Resource
Doctors House Calls
Doctors House Calls serves as a local, in-home clinical resource, conducting designated assessments and providing additional medical services when appropriate.
This relationship connects specialized dementia care coordination with professionals who can visit patients in their homes when in-person assistance is needed.
Who Is Eligible for GUIDE?
The GUIDE Model is intended for individuals with a confirmed dementia diagnosis who meet Medicare’s participation requirements.
Patients residing in memory care units or long-term nursing home settings are not eligible under current GUIDE requirements. Certain other residential care arrangements may qualify.
MedBetter Health determines eligibility and coordinates enrollment in accordance with CMS guidelines.
