Keep Your Patients. Extend Their Care Into the Home.

A Mobile Medical Resource for Patients Who Can’t Make It Into Your Office

Your patient may still consider you their doctor even when age, illness, mobility limitations or a recent hospitalization makes getting to your office difficult.

Doctors House Calls gives your practice another option.

We provide medical care in the patient’s home, helping you maintain continuity for patients who might otherwise go without routine care, rely on urgent care or the emergency department, or seek another provider simply because they can no longer get to you.

Support Your Patients Without Losing Them

A referral to Doctors House Calls does not have to mean giving up the patient.

DHC can provide in-home medical support when the patient cannot come to your office, while working cooperatively with your practice and other members of the patient’s care team.

Patients can continue seeing their regular physician, and Medicare beneficiaries retain the right to choose the physicians who treat them. DHC already describes its service model as either serving as the patient’s ongoing primary care provider or working with an existing physician.

When DHC Can Help Your Practice

Consider Doctors House Calls when a patient:

  • Becomes temporarily or permanently home-limited
  • Has difficulty arranging transportation
  • Misses appointments because of mobility or medical issues
  • Is discharged from the hospital or rehabilitation facility
  • Needs medical follow-up at home
  • Requires closer monitoring of a chronic condition
  • Needs medication management
  • Needs wound evaluation or treatment
  • Requires mobile labs, X-rays, ultrasound or other diagnostic services
  • Would benefit from medical evaluation in their home environment

Your Practice Stays Connected

Our role is to help close the gap between your office and the patient’s home.

When appropriate and authorized, DHC can communicate findings and care information back to the patient’s physician and coordinate with specialists, home health agencies, caregivers and other providers involved in the patient’s care.

DHC’s existing patient information specifically notes that reports of visits can be provided to physicians on request.

More Than a House Call

Doctors House Calls provides comprehensive home-based medical support, including:

  • Primary and follow-up care
  • Chronic care management
  • Transitional care management
  • Medication management and refills
  • Wound care
  • Mobile laboratory services
  • Diagnostic coordination
  • DME coordination
  • Home health coordination
  • Specialist referrals

A Resource for Your Hardest-to-Reach Patients

Some of the patients who need the most medical attention are also the ones who have the greatest difficulty getting into the office.

DHC gives your practice a way to continue supporting those patients rather than allowing transportation or mobility to become a barrier to care.
